Terms & Conditions – Physiotherapy Services
These Terms and Conditions (“Terms”) govern the physiotherapy services provided by [Clinic/Physiotherapist Name] (“we,” “our,” or “us”). By booking an appointment, attending an Initial Assessment, or receiving treatment, you (“client” or “you”) agree to these Terms. Please read them carefully before proceeding. If you have any questions, please raise them before entering into an agreement with us.
1. Initial Assessment
Before treatment begins, we will conduct an Initial Assessment to evaluate your needs and determine the most appropriate plan of care. The assessment generally lasts [xx minutes] and includes:
- A detailed discussion of your medical history, current concerns, and treatment goals.
- A physical evaluation of your condition.
- An objective examination requiring your cooperation and reasonable compliance with instructions.
- Development of a personalized treatment plan tailored to your requirements.
During this stage, you will also be asked to sign a Consent for Treatment Form and a Release of Information Form.
Please note:
- We reserve the right to decline treatment if the assessment indicates that physiotherapy is not appropriate or safe.
- If a significant time has passed between your assessment and your first treatment, we may require written confirmation that no major changes have occurred in your condition or request a new assessment.
2. Treatment Outcomes
- We do not guarantee specific results or outcomes, as these vary for each individual.
- Treatment effectiveness depends on multiple factors, including your condition, compliance, and overall health.
- We commit to delivering services with reasonable care and professional skill.
- Your treatment plan will be regularly reviewed and updated based on your progress.
3. Sessions & Home Assignments
- All sessions are scheduled and confirmed individually via phone, email, or in person.
- We may recommend home exercises or lifestyle modifications (“assignments”) to support your recovery. These are optional but highly encouraged for better outcomes.
- We reserve the right to end a session if you fail to follow reasonable instructions. In such cases, full session fees may still apply.
4. Fees & Payment Policy
- All session fees will be communicated and confirmed in advance.
- Payment is due at the start of each session unless otherwise agreed in writing.
- We accept payment by cash, cheque, debit/credit card, or net banking.
- If a third party (e.g., insurance provider) covers your fees, you remain liable for any unpaid or excess amounts not covered by them.
5. Cancellations & Late Arrivals
- Late Arrivals: If you arrive late, your session will still finish at the scheduled time. Extensions may be possible at our discretion and may incur additional charges.
- Cancellations: Appointments cancelled with less than 24 hours’ notice or missed without notice will be charged at the full session rate.
- Insurance providers generally do not reimburse late cancellations or no-show fees, meaning you will be personally responsible for such costs.
Clinic Cancellation Policy:
Occasionally, we may need to cancel or reschedule appointments (e.g., due to therapist illness, emergencies, or unavoidable delays). In such cases, you will not be charged and your session will be rescheduled at the earliest possible time.
6. Confidentiality & Data Protection
- All personal and medical information shared with us is treated as strictly confidential.
- We will not disclose your information to third parties without your prior consent unless required by law or necessary for your safety.
- We comply with the Data Protection Act and take all reasonable steps to store your information securely.
7. Liability & Indemnity
- While we strive to provide the best possible care, we are not liable for financial losses, personal dissatisfaction, or unmet expectations arising from treatment outcomes.
- Nothing in these Terms excludes or limits our liability for:
- Death or personal injury caused by our negligence.
- Fraud or fraudulent misrepresentation.
- By agreeing to these Terms, you indemnify us against any claims, damages, or legal costs arising from your breach of this agreement.
8. Early Termination of Services
We may terminate services with reasonable notice if:
- You behave inappropriately or refuse to cooperate.
- Your condition changes and physiotherapy is no longer suitable.
- A conflict of interest arises.
- External circumstances make treatment impractical or unsafe.
In such cases, refunds will be issued for any prepaid sessions not delivered.
9. Events Beyond Our Control
We are not responsible for delays, cancellations, or service interruptions caused by events outside our control, including but not limited to:
- Natural disasters (fire, flood, storm, earthquake).
- Strikes, lock-outs, or industrial disputes.
- Power outages, transport failures, or equipment breakdowns.
- Acts of God or other unforeseen emergencies.
